For new accounts, Ohio BMV lists the BMV 4890 new-account packet and BMV 4856 base-jurisdiction requirements. BMV reviews the application and establishes the account. Asal’s same-day service applies to preparing complete paperwork, not account approval or plate delivery.

What to bring for Ohio IRP
This checklist follows Ohio BMV’s IRP guidance and new-account forms, checked September 15, 2026. New accounts, renewals, and vehicle changes use different documents; BMV may request additional proof.
1. Owner and company records
Ohio driver license for the responsible owner or member, Ohio Secretary of State entity and DBA records where applicable.
2. Ohio base-jurisdiction proof
Complete BMV 4856 and provide the documents for either an established place of business or Ohio residency. See the two routes below.
3. Vehicle titles and details
Front and back of each title, including leased vehicles; VIN, unit number, year, make, axle count, purchase details, and requested registered weight.
4. Carrier and lease records
USDOT and tax-ID information for the motor carrier responsible for safety. Include BMV 4885 or the applicable authority lease. Leased vehicles may also need BMV 4845 or the titled owner’s power of attorney.
5. Tax and special vehicle documents
IRS-stamped or accepted Form 2290 Schedule 1 when required. Ohio BMV also lists an Ohio Highway Patrol HP 127B inspection for buses.
6. Fleet distance and application forms
Provide the applicable fleet distance by jurisdiction and weights. The BMV 4890 packet includes BMV 4837, 4838, and 4839; BMV also lists the PUC 3422 declaration for new applications. For an existing account, bring the account/fleet number, current cab cards, and transaction details.

Proof of your Ohio address
BMV 4856 requires the common owner/business records and one of these two proof routes:
Established place of business
Provide property ownership or a commercial lease, business hours and staffing evidence, the required Ohio employee withholding records, and business photographs described on BMV 4856. The location must have employees performing substantial trucking business duties.
Ohio residency
Provide two accepted proofs, such as a federal or Ohio income-tax return, property tax record or signed lease, a listed utility bill dated within 60 days, or a non-apportioned vehicle’s title, registration, and insurance together. Documents must meet BMV’s name and physical-address rules; the form explains the qualifying owner-name exception.
Use your qualifying physical Ohio address. A virtual office, mail service, or Asal’s address cannot serve as your IRP business address. The form also covers Ohio operations and record availability.

Is IRP account setup the same as getting apportioned plates?
A new IRP account application includes the business, fleet, and vehicle information needed for apportioned registration. Asal prepares the agreed paperwork; Ohio BMV reviews the application, determines registration charges, and issues the credentials and plates after its requirements are met. The $300 preparation fee does not include government registration or plate fees.
Can I use Asal’s office as my IRP business address?
No. Ohio BMV requires your qualifying physical Ohio business or residency address. A third-party preparer’s address, mail service, or virtual office cannot be used as the physical business address for your IRP account.
