National Registry II Makes Medical Certs Electronic

FMCSA's National Registry of Certified Medical Examiners entered its second phase (NR-II) on June 23, 2025. Medical exam results now transmit electronically to state licensing agencies, and the Motor Vehicle Record (MVR) became the official proof of medical certification.

What changed for drivers

Paper Medical Examiner's Certificates (MECs) are no longer the primary compliance document. After your exam, results flow electronically to your state DMV. Drivers should pull an MVR within a few days to confirm the med cert posted correctly—delays still happen when examiner data entry fails validation.

Examiner protocol updates

Certified medical examiners now use MUTCD-standard traffic sign images during vision and cognition screening portions of the exam. Examiners must be registered and active in NR-II; exams from delisted providers will not transmit.

Temporary waiver note

FMCSA issued transition guidance with a temporary waiver period for certain documentation gaps (extended through early 2026 in some regions). Do not rely on waivers for long-term compliance—treat MVR confirmation as mandatory in your DQ file audit.

Carrier DQ file impact

Update your driver qualification checklist: collect MVR after every med cert renewal, not just at hire. For Ohio-based fleets, align renewal timing with MCS-150 updates so SAFER reflects accurate power unit and driver counts.
